Join our dynamic team at Aledade as a Community Health Worker! In this crucial role, you will closely collaborate with medical providers, primary care teams, and social services agencies to enhance short-term care coordination focusing on Social Determinants of Health (SDOH). Your contribution will empower patients to improve their health and well-being through education and access to community resources.
π Primary Duties:
- Conduct targeted outreach to identify community resource needs in the patient population.
- Perform SDOH screenings and assist patients in developing self-management plans.
- Document all patient encounters and maintain comprehensive electronic files, ensuring effective documentation of activities, treatment plans, and patient outcomes.
- Facilitate communication between providers and patients while tracking compliance with care objectives.
- Bill for services provided, adhering to proper coding requirements.
- Create and maintain a community resource database tailored to patient populations.
- Actively participate in care team meetings and contribute to the patient care plan.
π Minimum Qualifications:
- High school diploma or GED.
- CHW certification or willingness to obtain upon hire.
- At least 1 year of relevant experience.
- Strong interpersonal skills with the ability to connect with a diverse community.
- Knowledge of community resources and health-related services.
- Proficiency in personal computer applications, including email and Excel.
π Preferred Skills:
- Familiarity with PCMH and PCMH-N care models.
- Experience with EHR systems or medical software.
- Billing and coding expertise.
